OUT works to secure the empowerment, human rights, access to equitable services and well-being of lesbian, gay, bisexual, and transgender plus communities in South Africa, the region and internationally.

OUT LGBT Well-being is currently recruiting qualified and experienced Case management Officer to support our USAID project Johannesburg.

This position will have a direct report to the Monitoring and Evaluation (M&E) Officer and will be for a fixed term.

Job purpose

Case management Officer will be responsible for ensuring proper linkage to care of all HIV infected patients and High-Risk negatives according to the Department of Health guidelines. Case management Officer work as part of a case management team to assist MSM service beneficiaries in enrolling in and accessing care and treatment services, while supporting them to identify and overcome barriers that interfere with achieving personal health-related goals.

Cost to Company (CTC)

R10 500,00 per month

Duties and Responsibilities include but are not limited to :

  1. Peer Navigation
  2. Support linkage to care of all HIV positive and High Risk Negative MSM
  3. Conduct tracing of patients who have missed their appointments or defaulted from care.
  4. Ongoing adherence and linkage support calls to build a relationship of the client, establish client linkage to designated referral points, GBV Support Network, Social Worker, and Hate Crimes
  5. Case Management Officer duties include referral to clinical, psychosocial, and other care and support services, encourage and support client’s appointments, adherence support, routine appointment reminders, follow-up for missed appointments, and tracking of those lost to follow-up.
  6. Maintain beneficiary confidentiality including in person, on the phone, through email and text messaging, and in the handling of hard copy documents.

 

2. Data Capturing

  1. Updating of Client Outcomes on the OUT-Information System, TIER.Net outcomes, client records and files and Ad Hoc M&E Tools as and when required i.e., surveys and HIVSS Registers

 

3. Case Management    

  1. Follow the Case Management SOP Bridging process to link newly diagnosed HIV positive people to the NIMART nurse so that they can initiate the client on ART.
  2. Participate as part of a case management team to assist HIV-positive service beneficiaries in enrolling in and accessing care and treatment services, while supporting them to identify and overcome barriers that interfere with achieving.
  3. Daily reports of all clients linked to care and on those not adherent to their treatment plan.
  4. Patient Management Counsellor are required to meet weekly to discuss individual cases, consider problems, and develop solutions to ensure optimal outcomes for each beneficiary.
  5. Assist with obtaining consent for Index Tracing and when needed, assist with Index Tracing
  6. Assist with tracing of results on NHLS.
  7. Escalate any challenges to the M&E Officer timeously.

 

Qualifications and experience

  1. Grade 12 Certificate
  2. Training in Basic HAST, HIV Counselling, Adherence counselling
  3. Knowledge of DoH HTS guidelines & protocols
  4. Experience working as an HCT counsellor.
  5. Must be able to work independently and as part of a team.
  6. Good oral and written communication skills
  7. Strong organizational skills, including task and time management, ability to multi-task, establish priorities, and meet deadlines, and work without supervision.
  8. Ability to work within a team that is diverse culturally, racially and which is accepting of gender diversity. 
  9. Be informative and knowledgeable and able to communicate on the topics of STIs, PrEP, Referral for HTS and treatment adherence.
