  • I Provided Psychosocial support and ongoing adherence couselling to HIV newly diagnosed patients.
  • Coordinated with health care facility to locate HIV newly identified cases required linkage to care and provided relevant information to patients around ART in clear manner.
  • I also reinforced adherence those who have interrupted on their treatment and virally unsuppressed patients.
  • Coordinated referrals to clinics, Conducted ongoing communication to provide support to assigned patients as per case management protocol.
  • Appointment reminders and tracked those who missed appointments (Early and Late missed) to minimize unconfirmed lost to follow up.
  • During this roles I was also exposed to tracing of patients and generating patient's files for tracing purposes and filling and Tie.net for generating line lists.
